Agassiz Valley Human Service Zone supports the elderly and/or disabled community that wants to continue to be successful in their own home environment. Join our team as a Direct Care Associate! Direct Care Associates provide direct service to the elderly and/or disabled individuals in their home to enable them to continue to safely reside in their home/community. In this role, you will be responsible for providing environmental services and personal care to eligible citizens. Services provided include laundry, shopping, housekeeping, meal preparation, transportation, bathing, hair care, etc. You will connect with local citizens to complete daily living activities with the goal of increasing their ability to be independent. Attention to detail is key because you will be responsible for accurately documenting activities through electronic visit verification and/or paper logs. This position will be based out of our Finley, North Dakota office location. An agency-issued vehicle is provided for business-related travel. As a Direct Care Specialist daily travel to client homes will be completed throughout Steele and Traill Counties. In addition, there may be some travel to rural Cass and rural Grand Forks counties based on client need. This position is part of the Agassiz Valley Human Service Zone Adult Services team. Team members participate in agency staffing and will receive training to maintain requirements as a Qualified Service Provider (QSP) provider/agency. To succeed in this position, you should have strong verbal and written communication skills, computer literacy, collaboration, and decision-making skills. You will have a passion for helping others and making a difference in our community. You will have strong attention to detail, be able to manage your time and priorities effectively and pride yourself on excellent communication skills, empathy, and being a team player. To be considered in this role:Must have a high school diploma or GED and (2) years of work experience providing direct care to special populations. Active CNA certification preferred. Would consider Direct Care Associate underfill, which requires a high school diploma or GED and (1) year of work experience providing direct care to special populations. Active CNA certification preferred. Selected candidate must successfully complete a standard criminal and child protection background checks and meet QSP requirements. Select candidate must have a valid driver's license with clean driving record.
